Introduction and objectives: Henoch Schönlein purpura (HSP) and Kawasaki disease (KD) are two main inflammatory diseases among childhood vasculitis. Considering the anti-inflammatory effects of 25-hydroxyvitamin D3, we decided to investigate the association of serum 25-hydroxy vitamin D3 level with the type and severity of these conditions. Materials and methods: The present study was performed as a historical cohort of 254 affected children with KD and HSP vasculitis. The required data were extracted, using a researcher-made questionnaire from patients’ electronic file, and then they were analyzed after collecting information of the patients. Results: In HSP group, 54% of participants were boys. Similarly, in KD group, boys were more affected than girls. The comparative 25-hydroxyvitamin vitamin D3 level in HSP patients with and without renal involvement (P=0.02), hematuria (P=0.14), and in two groups with and without heart disease, and also with and without coronary artery dilatation in KD patients (P<0.001) were significant. Discussion and conclusions: The findings showed that insufficient level of vitamin D3 were significantly associated with the exacerbation of complications of both diseases, and therefore it seems that vitamin D deficiency can be an effective predictive factor of severity in HSP and KD patients.(AU)


Introducción y objetivos: La púrpura de Henoch-Schönlein (HSP) y la enfermedad de Kawasaki (EK) son dos patologías inflamatorias principales entre las vasculitis infantiles. Teniendo en cuenta los efectos antiinflamatorios de la 25-hidroxivitamina D3, decidimos investigar la asociación del nivel sérico de esta con el tipo y la gravedad de dichas afecciones. Materiales y métodos: El presente estudio se realizó como una cohorte histórica de 254 niños afectados con vasculitis por EK y HSP. Los datos requeridos se extrajeron mediante un cuestionario elaborado por un investigador del expediente electrónico de los pacientes y se analizaron después de recopilar la información de los usuarios. Resultados: En el grupo HSP, 54 y 46% de los participantes eran niños y niñas, respectivamente. De manera similar, en el grupo KD, los varones se vieron más afectados. El nivel comparativo de 25-hidroxivitamina D3 en pacientes con HSP con y sin afectación renal (p = 0,02), hematuria (p = 0,14), y en dos grupos con y sin enfermedad cardiaca, y en dos con y sin dilatación de la arteria coronaria en usuarios con EK (p < 0,001) fueron significativos. Discusión y conclusiones: Los hallazgos mostraron que los niveles insuficientes de vitamina D se asociaron significativamente con la exacerbación de las complicaciones de ambas enfermedades, por lo que parece que la deficiencia de vitamina D puede ser un factor predictivo eficaz de la gravedad en pacientes con HSP y EK.(AU

Vitamin D (VD) deficiency has been associated with various tumors. However, the association between VD and skin cancer is controversial. Although in non-melanoma skin cancer, adequate or even high levels of VD can be associated with a higher risk of developing tumors, this could be biased by the direct association between sun exposure and VD levels. Regarding melanoma, results are contradictory. Most studies analyzed state that higher levels of VD could reduce the risk of melanoma, be associated with melanomas with better prognosis and with an enhanced antitumor response, and also with fewer adverse events associated with melanoma immunotherapy. However, prospective studies of adequate methodological quality are still needed to assess the association between VD levels and its supplementation and development/prognosis in skin cancer.

Objective: the aim of this study is to evaluate the relationship between serum vitamin D and B12 levels, nutritional levels, depression, and anxiety in adult cancer patients before and after chemotherapy. Methods: a case-controlled study was carried out on 44 patients who were diagnosed with cancer and applied to the Chemotherapy Unit for treatment (patient group, PG) and 44 volunteer individuals (control group, CG) with similar characteristics to the age and gender-matched patient group but with no diagnosis of cancer. Results: the average age of individuals in PG is 52.50 ± 12.21 years and for those in CG is 52.84 ± 10.98 years. Serum D and B12 levels in the first cure in individuals in PG are higher than in the last treatment (p > 0.05). It was determined that vitamin C taken with a daily diet reduces the risk of cancer (OR: 0.920, 95 % CI: 0.899-0.942, p = 0.042). No correlation was found between depression and anxiety scores of both groups and serum vitamin D and B12 levels (p > 0.05). It was determined that the Beck Anxiety Inventory (BAI) score increased with decrease in body mass index (BMI) (β = 0.311, p = 0.040) and serum vitamin B12 level (β = -0.406, p = 0.006). In addition, it was found that the increase in the Patient-Generated Subjective Global Assessment (PG-SGA) score, which reflects the nutritional status of cancer patients, worsened the level of anxiety (β = 0.389, p = 0.009). Conclusions: as stated in the findings of the study, chemotherapy treatment mediated the development of anxiety in cancer patients by changing the vitamin B12 levels and anthropometric characteristics with its negative effect on nutritional status. It should be ensured that cancer patients treated with chemotherapy follow a healthy and balanced diet plan that is suitable for their needs and has adequate vitamin and mineral content.(AU)


Objetivo: el objetivo de este estudio es evaluar la relación entre los niveles séricos de vitamina D y B12, el estado nutricional y el estado de depresión y de ansiedad antes y después de la quimioterapia en pacientes adultos con cáncer que están recibiendo quimioterapia. Métodos: se realizó un estudio de casos controlados en 44 pacientes diagnosticados de cáncer (grupo de pacientes, GP) que solicitaron tratamiento a la Unidad de Quimioterapia y 44 voluntarios sanos (grupo de control, GC) sin diagnóstico de cáncer y que tenían características similares al grupo GP en cuanto a edad y sexo. Resultados: la edad media de los individuos del GP fue de 52,50 ± 12,21 años, mientras que la del GC fue de 52,84 ± 10,98 años. Los niveles séricos de vitamina D y B12 en individuos del GP en el primer ciclo fueron más altos que en el último ciclo (p > 0,05). Se determinó que la vitamina C tomada en la dieta diaria reduce el riesgo de cáncer (OR: 0,920, IC del 95 %: 0,899-0,942, p = 0,042). No se detectó una correlación entre las puntuaciones de depresión y ansiedad de ambos grupos y los niveles séricos de vitamina D y B12 (p > 0,05). Se determinó que la puntuación Inventario de Ansiedad de Beck (BAI) aumentó con la disminución del índice de masa corporal (IMC) (β = 0,311, p = 0,040) y el nivel sérico de vitamina B12 (β = -0,406, p = 0,006). Background Vitamin D deficiency associates with the risk of developing many diseases, including cancer. At the molecular level, vitamin D appears to have an antineoplastic effect. However, the role of vitamin D deficiency in cancer pathogenesis remains unelucidated and numerous studies have resulted in discordant results. This study aimed to determine whether vitamin D deficiency during melanoma diagnosis increases the risk of developing non-cutaneous second primary cancers (SPC). Materials and methods A retrospective study on 663 patients diagnosed with melanoma between 1 January 2011 and 31 October 2022. The effect of each variable on the development of a subsequent non-cutaneous cancer was performed using Kaplan–Meier curves and differences were assessed by log-rank tests. Cox proportional hazard univariate and multivariate models were used to quantify the effect of each variable in the time to develop a non-cutaneous neoplasia. Results Out of 663 patients, 34 developed a non-cutaneous SPC. There was no statistically significant association between vitamin D levels and non-cutaneous SPC development (log-rank, p=0.761). Age>60 years, stage III/IV, and nodular melanoma subtype were significantly associated with the development of a SPC. After multivariate analysis, only age>60 years (HR 3.4; HR CI 95%: 1.5–7.6) and nodular melanoma subtype (HR 2.2; HR CI 95%: 1.0–4.8) were included in the final model. Conclusions Our results suggest that vitamin D deficiency is not associated with an increased risk of developing non-cutaneous SPC in melanoma patients. However, age over 60 years and nodular melanoma subtype increase the risk for non-cutaneous SPC development (AU)


Antecedentes El déficit de vitamina D se asocia con un mayor riesgo de padecer varias enfermedades, incluido el cáncer. Molecularmente, esta parece tener un efecto antineoplásico. Sin embargo, el papel que juega en la patogénesis del cáncer no está bien esclarecido y hay resultados dispares en los estudios publicados. El objetivo del presente fue determinar si unos niveles de vitamina D deficientes en el momento del diagnóstico del melanoma aumentaba el riesgo de desarrollar un cáncer no cutáneo (CNC). Material y método Se diseñó un estudio retrospectivo de 663 pacientes diagnosticados de melanoma entre el 1 de enero de 2011 y el 31 de octubre de 2022. El efecto de cada una de las variables seleccionadas en el desarrollo de un CNC durante el seguimiento tras el diagnóstico del melanoma se realizó mediante el estudio de supervivencia con el método de Kaplan-Meier y las diferencias se evaluaron con la prueba de los rangos logarítmicos. Se elaboraron modelos uni y multivariados de riesgos proporcionales de Cox para cuantificar el efecto de cada valor de las variables de estudio en el tiempo para desarrollar un CNC. Resultados De los 663 pacientes, 34 desarrollaron un CNC tras el melanoma. No hubo diferencias estadísticamente significativas entre los grupos definidos por los niveles de vitamina D (log-rank, p = 0,761). Sin embargo, una edad > 60, el estadio III/IV, y el tipo nodular se asociaron significativamente al desarrollo de un CNC. Tras el análisis multivariado, solo la edad > 60 (hazard ratio [HR] 3,4; intervalo de confianza [IC] 95% HR:1,5-7,6) y el subtipo nodular de melanoma (HR 2,2; IC 95% HR:1,0-4,8) se mantuvieron en el modelo predictivo final. RESULTS: Out of 663 patients, 34 developed a non-cutaneous SPC. There was no statistically significant association between vitamin D levels and non-cutaneous SPC development (log-rank, p=0.761). Age>60 years, stage III/IV, and nodular melanoma subtype were significantly associated with the development of a SPC. After multivariate analysis, only age>60 years (HR 3.4; HR CI 95%: 1.5-7.6) and nodular melanoma subtype (HR 2.2; HR CI 95%: 1.0-4.8) were included in the final model. CONCLUSIONS: Our results suggest that vitamin D deficiency is not associated with an increased risk of developing non-cutaneous SPC in melanoma patients. However, age over 60 years and nodular melanoma subtype increase the risk for non-cutaneous SPC development.

ABSTRACT Objective: To assess mothers' knowledge on sun exposure related to serum vitamin D levels in the neonatal period. Methods: Observational, analytical and cross-sectional study, carried out from August 2020 to May 2021 through a questionnaire directed to mothers of newborns, in a maternity hospital in Southern Brazil. Results: From 141 interviewees, 132 (93.6%) believe it is important to expose the neonate to sun, 101 (71.6%) think this exposure can increase vitamin D levels, 86 (61%) received such information from a doctor, 108 (76.6%) believe there are no risks of sun exposure, 88 (62.4%) claim it isn´t necessary to use any kind of protection, 96 (68.1%) said that only exposure to the sun was necessary to maintain adequate levels of vitamin D during the neonatal period. Only two mothers (1.4%) claim that you should not exposure the neonate to the sun, and only one (0.7%) stated that sun expose can cause skin problems. Conclusions: Most mothers lack satisfactory knowledge about sun exposure related to serum vitamin D levels in the neonatal period. The need to inform and clarify the population about sun exposure during this period is remarkable, in addition to disseminating the proper way to maintain serum levels of vitamin D.


RESUMO Objetivo: Avaliar o conhecimento das mães acerca da exposição solar relacionada com níveis séricos de vitamina D no período neonatal. Métodos: Estudo observacional, analítico e transversal, realizado de agosto de 2020 a maio de 2021 por meio de questionário dirigido às mães de recém-nascidos, em uma maternidade no sul do Brasil. Resultados: De 141 entrevistadas, 132 (93,6%) acreditam ser importante expor o lactente ao sol no primeiro mês de vida, 101 (71,6%) acham que essa exposição aumenta os níveis de vitamina D, 86 (61,0%) receberam tal informação de um médico, 108 (76,6%) acreditam que expor o neonato ao sol não causa riscos para a saúde, 88 (62,4%) acham que não é necessário usar proteção contra radiação solar ao expor o neonato ao sol, e 96 (68,1%) afirmaram que apenas a exposição ao sol basta para manter os níveis adequados de vitamina D durante o período neonatal. Apenas duas mães (1,4%) afirmaram que não se deve expor o neonato ao sol e uma (0,7%) que a exposição solar pode causar problemas de pele. Conclusões: A maioria das mães não possui conhecimento satisfatório acerca da exposição solar relacionada aos níveis séricos de vitamina D no período neonatal. É notável a necessidade de informar e esclarecer a população sobre a exposição solar nesse período, além de disseminar a maneira adequada de manter os níveis séricos de vitamina D.

Recent scientific evidence suggests a close relationship between estrogen deficiency and vitamin D- related genes. Estrogen and vitamin D were involved with alterations in odontogenesis and tooth eruption process. Objective: The aim of the present study was to evaluate the influence of estrogen deficiency on the expression of genes related to the activation and degradation of vitamin D in the odontogenic region of incisors in a murine model. Material and Methods: This is an experimental clinical study that used female Wistar Hannover rats. The animals were randomly divided into two groups according to the intervention received: Hypoestrogenism Group ­ animals submitted to estrogen deficiency by ovariectomy surgery and Control Group ­ animals submitted to sham surgery. Surgical intervention was performed in the prepubertal period; the animals were followed throughout the pubertal period. After euthanasia, the hemimandibles were removed to evaluate the mRNA expression of the vitamin D-related genes AMDHD1, CYP24A1, NADSYN1 and SEC23A in the odontogenic region of incisors through real time PCR. Student's t test was used to compare means. Kruskal-Wallis test and Dunn's posttest were also used. The level of significance was 5%. Results: SEC23A was overexpressed in the estrogen deficiency condition in the odontogenic region (p=0.021). Conclusion: Estrogen deficiency may influence the expression of the SEC23A gene involved in the activation and degradation of vitamin D in the odontogenic region of incisors in a murine model(AU)


Evidências científicas recentes sugerem uma estreita relação entre a deficiência de estrógeno e os genes relacionados à vitamina D. O estrógeno e a vitamina D estão envolvidos com alterações na odontogênese e no processo de erupção dentária. Objetivo: O objetivo do presente estudo foi avaliar a influência da deficiência de estrógeno na expressão de genes relacionados à ativação e degradação da vitamina D na região odontogênica de incisivos em modelo murino. Material e Métodos: Trata-se de um estudo clínico experimental que utilizou ratas Wistar Hannover fêmeas. Os animais foram divididos aleatoriamente em dois grupos de acordo com a intervenção recebida: Grupo Hipoestrogenismo ­ animais submetidos à deficiência de estrógeno pela cirurgia de ovariectomia e Grupo Controle ­ animais submetidos à cirurgia simulada. A intervenção cirúrgica foi realizada no período pré-púbere; os animais foram acompanhados durante todo o período puberal. Após a eutanásia, as hemimandíbulas foram removidas para avaliar a expressão de mRNA dos genes AMDHD1, CYP24A1, NADSYN1 e SEC23A, relacionados à vitamina D, na região odontogênica de incisivos por meio de PCR em tempo real. O teste t de Student foi utilizado para comparar as médias. Também foram utilizados o teste de Kruskal-Wallis e o pós-teste de Dunn. O nível de significância foi de 5%. Resultados: SEC23A foi superexpresso na condição de deficiência de estrógeno na região odontogênica (p=0,021). Conclusão: A deficiência de estrógeno pode influenciar a expressão do gene SEC23A envolvido na ativação e degradação da vitamina D na região odontogênica de incisivos em modelo murino (AU)

Vitamin D receptor polymorphisms are associated with different diseases such as rheumatoid arthritis and diabetes. The aim of this study is to investigate the possible association of two identified mutations ApaI (rs7975232) and TaqI (rs731236). Eighty-nine healthy individuals and Fifty-six Type 2 Diabetic (T2D) patients were investigated using RFLP technique for genotyping and haplotyping as well. The distribution of Apal genotypes was not statistically significant among the control (P=0.65) as well as for diabetic patients (P=0.58). For Taql allele frequencies of T allele was 0.61 where of G allele was 0.39. The frequency distribution of Taql genotypes was not statistically significant among the control (P=0.26) as well as diabetic patients (P=0.17). Relative risk of the allele T of Apa1 gene is 1.28 and the odds ratio of the same allele is 1.53, while both estimates were < 1.0 of the allele G. Similarly, with the Taq1 gene the relative risk and the odds ratio values for the allele T are 1.09 and 1.27 respectively and both estimates of the allele C were 0.86 for the relative risk and 0.79 for the odds ratio. The pairwise linkage disequilibrium between the two SNPs Taq1/apa1 was statistically significant in control group (D = 0.218, D' = 0.925 and P value < 0.001) and similar data in diabetic groups (D = 0.2, D' = 0.875 and P value < 0.001). These data suggest that the T allele of both genes Apa1 and Taq1 is associated with the increased risk of type 2 diabetes. We think that we need a larger number of volunteers to reach a more accurate conclusion.


Resumo Várias razões podem estar subjacentes ao aumento dramático da diabetes mellitus tipo 2. Um desses motivos é a base genética e variações. Os polimorfismos do receptor da vitamina D estão associados a diferentes doenças, como artrite reumatoide e diabetes. O objetivo deste estudo é investigar a possível associação de duas mutações identificadas ApaI (rs7975232) e TaqI (rs731236). Antecedentes y objetivo El solapamiento clínico y bioquímico de diversas enfermedades del metabolismo fosfocálcico puede conllevar un erróneo diagnóstico y su consecuente abordaje clínico. Un ejemplo es el seudohipoparatiroidismo, que puede confundirse con el raquitismo dependiente de vitamina D (VDDR1) si no se hacen las determinaciones bioquímicas adecuadas. Pacientes y métodos Dos parejas de hermanos, de familias independientes, fueron diagnosticados clínicamente en la adolescencia de seudohipoparatiroidismo al presentar hipocalcemia, niveles elevados de hormona paratiroidea y valores normales o elevados de fósforo. Tras descartar alteraciones en GNAS, se realizó un estudio, mediante secuenciación masiva, de genes asociados a otros diagnósticos diferenciales. Resultados Se identificaron 2variantes genéticas en el gen CYP27B1 potencialmente asociadas con el fenotipo. Variantes patogénicas en este gen se asocian con VDDR1A. La reevaluación clínica-bioquímica de los pacientes confirmó dicho diagnóstico y se adecuó el tratamiento. Conclusiones Si bien la VDDR1A es un trastorno del metabolismo de diagnóstico infrecuente en la edad adulta, en casos de hipocalcemia con valores elevados de PTH es relevante la determinación de las formas 1,25(OH)2D3 y 25(OH)D3 de la vitamina D para alcanzar un diagnóstico correcto (AU)


Background and objective The clinical and biochemical overlap of various pathologies of phosphocalcic metabolism can lead to misdiagnosis and consequent clinical management. One example is pseudohypoparathyroidism, which can be confused with vitamin D-dependent rickets (VDDR1) if appropriate biochemical determinations are not performed. Patients and methods Two pairs of siblings, from independent families, were clinically diagnosed in adolescence with pseudohypoparathyroidism due to hypocalcaemia, elevated parathyroid hormone levels and normal or elevated phosphorus values. After ruling out alterations in GNAS, a massive sequencing study of genes associated with other differential diagnoses was carried out. Results Two genetic variants in the CYP27B1 gene potentially associated with the phenotype were identified. Pathogenic variants in this gene are associated with VDDR1A. Clinical-biochemical re-evaluation of the patients confirmed this diagnosis and treatment was adapted. Conclusions Although VDDR1A is an infrequently diagnosed pathology in adulthood, in cases of hypocalcaemia with elevated PTH values, determination of the 1,25(OH)2D3 and 25(OH)D3 forms of vitamin D is relevant to reach a correct diagnosis (AU)

Objetivo. Determinar los niveles plasmáticos de vitamina D y su relación con la ocupación, procedencia y los valores de calcio, leucocitos, hemoglobina y plaquetas en adultos mayores de Lima Metropolitana. Método. Estudio observacional descriptivo de corte transversal, participaron 100 adultos mayores de uno u otro sexo de Lima Metropolitana, ellos fueron reclutados durante las estaciones de invierno-primavera del 2022, los niveles de vitamina D se categorizó como suficiente, insuficiente y deficiente, las concentraciones séricas de vitamina D se midieron por radioinmunoensayo. La relación de variables se realizó con el coeficiente de correlación de Spearman y regresión logística. Resultados. La edad media fue 69.6 años, el 71%, fueron del sexo femenino, la concentración media de vitamina D fue 36.56 ng/ml, el 13% tuvieron niveles de deficiente, y el 32% de insuficiente y el 53% suficientes. Según sexo, el 77,7% de los que tuvieron niveles de deficiente/insuficiente fueron mujeres. En el análisis bivariado no hubo correlación entre los valores de vitamina D con calcio, hemoglobina leucocitos y plaquetas, los que procedieron de distritos de menor temperatura tuvieron 2,25 veces más riesgo de tener niveles insuficientes/deficiente de vitamina D. Conclusiones. El 45% de los adultos mayores tuvieron niveles deficientes/insuficiente de vitamina D, siendo más frecuente en las mujeres y de los procedentes de distritos de menor promedio de temperatura.


Objective. To determine the plasmatic levels of vitamin D and its relationship with the occupation, origin and the values of calcium, leukocytes, hemoglobin and platelets in older adults of Metropolitan Lima. Methods Observational descriptive cross-sectional study, 100 older adults of either sex from Metropolitan Lima participated, they were recruited during the winter-spring seasons of 2022, vitamin D levels were categorised as sufficient, insufficient, and deficient, serum vitamin D concentrations were measured by radioimmunoassay. The relationship of variables was established with the Spearman correlation coefficient and logistic regression. Results. The mean age was 69.6 years, 71% were women, the mean vitamin D concentration was 36.56 ng/ml, 13% had deficient levels, 32% were insufficient and 53% sufficient. By sex, 77.7% of those with deficient/insufficient levels were women. In the bivariate analysis, there was no correlation between vitamin D values with calcium, hemoglobin, leukocytes and platelets; those who came from districts with lower temperatures had a 2.25 times greater risk of having insufficient/deficient levels of vitamin D. Conclusions. 45% of older adults had deficient/insufficient levels of vitamin D, more frequent in women and those coming from districts with lower average temperatures.

Introduction: obesity childhood is related to vitamin D deficiency. The aim of this study was to compare vitamin D status between adolescentswith obesity living in an urban area and in a rural area. We hypothesized that environmental factors would be decisive in reducing the bodycontent of vitamin D in patients with obesity.Methods: a cross-sectional clinical and analytical study (calcium, phosphorus, calcidiol and parathyroid hormone) was carried out in a group of259 adolescents with obesity (BMI-SDS > 2.0), 249 adolescents with severe obesity (BMI-SDS > 3.0) and 251 healthy adolescents. The placeof residence was categorized as urban or rural. Vitamin D status was defined according to the US Endocrine Society criteria.Results: vitamin D deficiency was significantly higher (p < 0.001) in severe obesity (55 %) and obesity groups (37.1 %) than in the control group(14 %). Vitamin D deficiency was more frequent in severe obesity (67.2 %) and obesity groups (51.2 %) living in urban areas than in those livingin rural areas (41.5 % and 23.9 %, respectively). The patients with obesity living in urban residence did not present significant seasonal variationsin vitamin D deficiency in contrast to those patients with obesity living in rural residence.Conclusions: the most probable mechanism for vitamin D deficiency in adolescents with obesity, rather than altered metabolic is the environmentalfactors (sedentary lifestyle and lack of adequate sunlight exposure). Introducción: la obesidad está relacionada con la deficiencia de vitamina D. El objetivo del presente estudio fue comparar el estado de vitaminaD entre adolescentes con obesidad con residencia urbana y rural. Se hipotetiza que los factores ambientales serían determinantes en la reduccióndel contenido corporal de vitamina D en pacientes con obesidad.Métodos: estudio transversal clínico y analítico (calcio, fósforo, calcidiol y PTH) en un grupo de 259 adolescentes con obesidad (IMC-SDS >2,0), 249 adolescentes con obesidad severa (IMC-SDS > 3,0) y 251 adolescentes sanos. El estado de vitamina D se definió de acuerdo con loscriterios de la Sociedad Americana de Endocrinología.Resultados: la deficiencia de vitamina D fue superior (p < 0,05) en los grupos de obesidad severa (55 %) y obesidad (37,1 %) respecto al grupode control (14 %). La prevalencia de deficiencia de vitamina D fue superior (p < 0,05) en los grupos de obesidad severa (67,2 %) y obesidad(51,2 %) con residencia urbana respecto a los que vivían en áreas rurales (41,5 % y 23,9 %, respectivamente). Los pacientes con obesidad yobesidad severa con residencia urbana no presentaban variaciones estacionales en la deficiencia de vitamina D en contraste con los pacientescon residencia rural.Conclusiones: el mecanismo más probable para la deficiencia de vitamina D en adolescentes con obesidad, más que alteraciones metabólicas,serían los factores ambientales (sedentarismo y falta de exposición solar adecuada). Objective: to explore the association between serum vitamin D level and the occurrence and pathological grade of gastric cancer. Material and methods: search PubMed, Embase, Web of Science, Cochrane and Chinese database; all articles about the association between serum vitamin D levels and gastric cancer published before July 2021. Results: 10 trials with 1159 cases of gastric cancer patients and 33,387 cases of regular control patients were analyzed. The serum vitamin D level of the gastric cancer group (15.56 ± 7.46 ng/ml) was lower than in the control group (17.60 ± 1.61 ng/ml), and the difference wasstatistically significant. The patients with gastric cancer, clinical stage III/IV (16.19 ± 8.04 ng/ml) had lower vitamin D levels than those with stageI/II (19.61 ± 9.61 ng/ml), and the patients with low differentiation of gastric cancer (17.5 ± 9.5 ng/ml) had lower levels than those with well- ormoderately-differentiated cancer (18.04 ± 7.92 ng/ml). The patients with lymph node metastasis (19.41 ± 8.63 ng/ml) had lower vitamin Dlevels than the patients without lymph node metastasis (20.65 ± 7.96 ng/ml), and the difference was statistically significant. Conclusion: vitamin D levels were negatively associated with gastric cancer. Vitamin D levels were significantly associated with different clinicalstages, degrees of differentiation, and lymph node metastasis, suggesting that low vitamin D levels might predict poor prognosis in gastric cancer. Objetivo: investigar la asociación entre los niveles de vitamina D en suero y la carcinogénesis gástrica y su clasificación patológica. Material y métodos: se buscaron en las bases de datos PubMed, Embase, Web of Science, Cochrane y China todos los artículos sobre la asociación entre los niveles séricos de vitamina D y el cáncer gástrico publicados antes de julio de 2021.Resultados: se analizaron diez datos de 1159 pacientes con cáncer gástrico y 33.387 pacientes normales de control. El nivel de vitamina D en suero del grupo (15,56 ± 7,46 ng/ml) de cáncer gástrico era inferior al del grupo de control (16,19 ± 8,04 ng/ml), y la diferencia era estadísticamente significativa. En los pacientes con cáncer gástrico en estadio clínico III/IV (16,19 ± 8,04 ng/ml) era inferior al de los pacientes en estadio I/II (19,61 ± 9,61 ng/ml) y en los pacientes con cáncer de estómago poco diferenciado (17,5 ± 9,5 ng/ml) era menor que en los pacientes con cáncer bien o moderadamente diferenciado (18,04 ± 7,92 ng/ml); en los pacientes con metástasis en ganglios linfáticos (19,41 ± 8,63 ng/ml)era inferior al de los pacientes sin metástasis en ganglios linfáticos (20,65 ± 7,96 ng/ml), siendo la diferencia estadísticamente significativa. Conclusión: los niveles de vitamina D se correlacionaron negativamente con el cáncer gástrico. Los niveles de vitamina D se asociaron significativamente a los diferentes estadios clínicos, el grado de diferenciación y la metástasis en ganglios linfáticos, lo que sugiere que los niveles bajos de vitamina D pueden ser un factor de predicción de mal pronóstico en el cáncer gástrico. Introduction Vitamin D deficiency has been proposed to confer susceptibility to acquiring tuberculosis infection by impairing the innate immune response. Methods In an exploratory study, we examined whether the levels of 25-hydroxyvitamin D3 (25(OH)D3) in serum, and cathelicidin – an antimicrobial peptide-induced under calcitriol – in the nasal fluid, would associate with the risk of acquiring tuberculosis infection. Results Within a prospective cohort of 231 tuberculosis household contacts tested with repeated interferon-gamma release assays, we serially analyzed all the uninfected contacts acquiring tuberculosis infection at follow-up (“converters”, n=18), and an age and sex-matched control group of contacts not acquiring tuberculosis infection (“non-converters”, n=36). The median levels of serum 25(OH)D3 did not differ between convertors and non-converters at baseline (14.9 vs. 13.2 ng/ml, p=0.41), nor at follow-up (19.0 vs 18.6ng/ml, p=0.83). Similarly, cathelicidin levels did not differ between both groups. Conclusion These data argue against a major role for hypovitaminosis D in tuberculosis infection susceptibility (AU)


Introducción Se ha propuesto que la deficiencia de vitamina D, al afectar la respuesta inmunitaria innata, aumentaría la susceptibilidad de adquirir una infección por Mycobacterium tuberculosis. Métodos En un estudio exploratorio, examinamos si los niveles de 25-hidroxivitamina D3 (25(OH)D3) y de catelicidina (péptido antimicrobiano inducido bajo calcitriol) en suero y fluido nasal, respectivamente, se asocian con el riesgo de contraer una infección tuberculosa. Resultados En una cohorte prospectiva de 231 contactos intradomiciliarios de tuberculosis en los que se realizaron ensayos de liberación de interferón-gamma en forma seriada, estudiamos a todos los contactos no infectados que adquirieron la infección al seguimiento («conversores», n=18), y a un grupo control pareado por edad y sexo que no adquirió la infección tuberculosa («no conversores», n=36). La mediana de los niveles séricos de 25(OH)D3 no difirió entre convestores y no conversores al inicio del estudio (14,9 vs. 13,2 ng/ml, p=0,41), ni al seguimiento (19,0 vs. 18,6 ng/ml, p=0,83). Igualmente, los niveles de catelicidina nasal no difirieron entre ambos grupos. Conclusión Estos resultados no apoyan la existencia de un papel significativo de la hipovitaminosis D en la susceptibilidad a la infección por tuberculosis (AU)

Los trastornos autoinmunes representan una familia de al menos 80 condiciones diferentes que surgen de una respuesta aberrante del sistema inmunológico resultando finalmente en la destrucción de tejidos y órganos específicos del cuerpo. Es importante destacar que durante las últimas tres décadas los estudios epidemiológicos han proporcionado evidencia de un aumento constante en la incidencia y prevalencia de trastornos autoinmunes. En los últimos años, varios estudios han demostrado que la vitamina D y los ácidos grasos poliinsaturados (AGPs) omega-3 ejercen propiedades inmunomoduladoras y antiinflamatorias sinérgicas que pueden aprovecharse positivamente para la prevención y el tratamiento de trastornos autoinmunes. En este sentido, el reciente ensayo clínico denominado VITAL (ensayo de vitamina D y omega 3); un estudio a gran escala, aleatorizado, doble ciego, controlado con placebo encontró que la suplementación conjunta de vitamina D y AGPs omega-3 (VIDOM) puede reducir la incidencia de enfermedades autoinmunes. En esta revisión de la literatura, resumimos los mecanismos moleculares detrás de las propiedades inmunomoduladoras y antiinflamatorias de la vitamina D y los AGPs omega-3, así como la posible interacción bidireccional entre el metabolismo de la vitamina D y el metabolismo de los AGPs omega-3 que justifica la co- suplementación VIDOM en trastornos autoinmunes(AU)


Autoimmune disorders represent a family of at least 80 different conditions that arise from an aberrant immune system response, which ultimately results in the destruction of specific body tissues and organs. It is important to highlight that during the last three decades epidemiological studies have provided evidence of a steady increase in the incidence and prevalence of autoimmune disorders. In recent years, several studies have shown that vitamin D and omega-3 polyunsaturated fatty acids (PUFAs) exert synergistic immunomodulatory and anti-inflammatory properties that can be positively harnessed for the prevention and treatment of autoimmune disorders. In this sense, the recent clinical trial called VITAL (Vitamin D and Omega 3 trial) - a large, randomized, double-blind, placebo- controlled study - found that co-supplementation of vitamin D and omega-3 PUFAs (VIDOM) can reduce the incidence of autoimmune diseases. In this literature review, we summarize the molecular mechanisms behind the immunomodulatory and anti-inflammatory properties of vitamin D and omega-3 PUFAs, as well as the possible bidirectional interaction between vitamin D metabolism and omega-3 PUFA metabolism that justifies VIDOM co- supplementation in autoimmune disorders(AU)

Introducción: El déficit de vitamina D y la diabetes son dos situaciones prevalentes en todas las edades, regiones geográficas y niveles socioeconómicos. La presencia de receptores para 1,25(OH)2D y la existencia de la enzima 1-α-hidroxilasa en la célula beta ­que permite la síntesis del metabolito activo­ sugieren que la vitamina D juega un papel importante en dichas células y que su deficiencia podría ser un factor capaz de acelerar el inicio y la evolución de la enfermedad. Objetivos: Se realizó una revisión del tema vitamina D y Diabetes. A su vez, se analizó el rol de la vitamina D en la insulinorresistencia y en la DM2, así como en la autoinmunidad y la DM1. También se indagó en el estado actual sobre los efectos de la suplementación con vitamina D en la prevención, el control glucémico y la evolución de las complicaciones asociadas a esta enfermedad. Materiales y Métodos: Se realizó una búsqueda bibliográfica utilizando los buscadores PubMed, MEDLINE, Cochrane, Research Gate. Los criterios de búsqueda fueron "vitamin D", "vitamin D and diabetes", "vitamin D and supplementation". Conclusiones: Existe suficiente evidencia acerca de que los niveles séricos de 25(OH)D deberían alcanzar valores entre 30 y 50 ng/ml para influir en las funciones metabólicas. Se enfatiza en la importancia de incluir el dosaje de 25(OH)D en el control clínico de rutina. Teniendo en cuenta la escasa distribución de la vitamina D en los alimentos naturales, el bajo consumo de los alimentos fuente en la población argentina y las recomendaciones actuales de limitar la exposición al sol por el cáncer de piel, emerge como necesario contar con alimentos fortificados de consumo masivo, además de la leche


Introduction: Vitamin D deficiency and diabetes are two prevalent situations in all ages, geographic regions and socioeconomic levels. The presence of receptors for 1,25(OH)2D and the existence of the enzyme 1-α-hydroxylase in the beta cell ­which allows the synthesis of the active metabolite­ suggest that vitamin D plays an important role in these cells and that vitamin deficiency could be a factor that can accelerate the onset and progression of the disease. Objectives: The subject vitamin D and Diabetes was review. The role of vitamin D in insulin resistance and DM2, as well as autoimmunity and DM1, were analyze. We also evaluated the effects of vitamin D supplementation on prevention, glycemic control and evolution of associated complications. Materials and methods: A bibliographic search was carried out using the search engines PubMed, MEDLINE, Cochrane, Research Gate. The search criteria were "vitamin D", "vitamin D and diabetes", "vitamin D and supplementation". Conclusions: There is sufficient evidence that blood levels of 25(OH)D should reach values between 30 and 50 ng/ml to influence metabolic functions. The importance of including 25(OH)D dosage in routine clinical control is emphasized. Taking into account the scarce distribution of vitamin D in natural foods, the low consumption of source foods in the Argentine population and the current recommendations to limit sun exposure due to skin cancer, it emerges as necessary to have fortified foods for mass consumption, in addition to milk
